We currently have an exciting new opportuniy for you to bring your excellent work ethic as a dedicated Social Worker in Population and Public Health in the Fraser Health area. As we enter the recovery phase, the response to the COVID-19 pandemic will require ongoing vigilance, especially in the case and contact management – also known as contact tracing.

 

Build on your education & career experience as you:

  • Conduct social work and assessments by interviewing the client and/or their family, acquiring relevant information, capturing social data and interpreting the data using a variety of tools.
  • Provide clinical counseling services to patients and families regarding identified problems, personal concerns and recommended public health measures.
  • Consult and collaborate with the interdisciplinary teams regarding client challenges, resources and community supports.
  • Maintain patient records by detailing assessments, charting information and preparing progress notes and reports in accordance with established standards, policies, procedures and current regulatory requirements.
  • Participate in department or program-sponsored research through various methods such as literature searches, coordinating surveys and analyzing data as required.
  • Participate in program and community resource development activities by working with community service agencies and the hospital in order to respond to identified gaps in service and recognized patient needs.

To join our team you have:

  • Current registration with the BC College of Social Workers.
  • Two (2) years' recent related experience in a complex health care environment such as a community or home health setting, or an equivalent combination of social work education, training and experience.
  • Valid BC Driver's License and access to personal vehicle for business-related purposes.
