What services do you offer?

We offer both short and long-term services.

Short-term support helps you recover from a hospital procedure or condition. Long-term programs provide ongoing care that will give you the independence you need to remain at home comfortably.

Our Home Health staff will work with you to determine your best care options. Our approach to meeting your health care needs is guided by the following principles:

• Everyone should have the information they need to make decisions about lifestyle and care
• Services will complement and supplement, but not replace, your efforts to care for yourself
• You have the right to make your own care decisions, including the right to live with risk
• Services will promote your well-being, dignity and independence
