Fraser Health collaborates closely primary care providers to develop new systems and programs to support high-quality patient care in the community.

• 1435+ Family Physicians and Specialists and 12 Nurse Practitioners in Fraser Health, in 20 communities
• Also working in Primary Care: public health staff, community nurses, midwives, pharmacists, mental health professionals, clinical counsellors, physiotherapists, home and community care workers, mental health specialists, dieticians, community organizations and more

Integration of primary and community care

BC’s health authorities are in the process of building an ‘integrated system of primary and community care,’ so that British Columbians to have the majority of their needs met by high-quality, community-based health care and support services.

One of the ways we will achieve this is through the creation of integrated health networks in all of our communities over the next five years. This involves building a team of integrated professionals, who come together with family doctors to support people who are living with complex or chronic health conditions.  This work also involves:
